COVID-19 Community Concern Reporting Form

Hello Students,

Can you believe we are almost at the end of week 5 of classes? I’m so proud of all of you for all you are doing to keep the spread of COVID-19 at bay so we can continue to be in community together. Please stay vigilant and continue the great work!

The University has received inquiries from various community members about how to report COVID-19 concerns — items such as face covering usage and physical distancing — about other community members to the University. We would encourage you to continue to discuss these concerns directly with those individuals as this likely will have the greatest impact. That being said, we have also set up a reporting tool for the community to use as well.

In order to assist the University in maintaining a safe and healthy campus community, University community members have the option to report COVID-19 related concerns to the University administration via the COVID-19 Community Concern Reporting Form. After receiving the concern, University staff will review and investigate. Based on the investigation, the concern will be addressed as the circumstances deem appropriate.

Individuals creating a report may do so anonymously. Please note that anonymous reports will be honored, but it may limit the scope of our response to holding individual(s) accountable. Further, if the individual creating the report shares their contact information, you may not receive a response from the University in regards to your complaint. This will be considered based on the circumstances and nature of the report.

In closing, I would also encourage you to continue to find ways to be in community and to take care of yourselves. I know this “new normal” is creating some social challenges that are forcing us all to be more creative in ways to safely engage our campus community. Getting out of your room, meeting new people, and hanging out and talking with friends are all things that we want you to be able to do, but we want you do it in a safe way. Engaging with your community is important and helps significantly with your overall well-being. The COVID-19 pandemic has created many challenges to interacting with others safely, but there are safe opportunities to engage with student organizations, attend fraternity and sorority events, take a fitness classes, attend residence hall or University Programming Council events (to name a few). You can check out the weekly “What’s Up Wednesday” email for upcoming campus events and details. We should be approaching this academic semester as a marathon, not a sprint. So, be sure to take care of yourselves and others along the way.
