All clinical paperwork is done in Bridges.  Please do it as soon as it is assigned as everything needs to be processed before you can pick up your badge.  Do all items, but please do the EHRAA and the quiz first.

EHRAA.  This form can be a little tricky, so you want to make sure to complete it, click on the side of the document so you aren’t in a text box, save it to your computer, make sure the information saves, and then upload it to Bridges.  Please complete information on all three pages.  On page three, for Select a Role choose Job code 5127:  Student Nurse Level II and under Specify Role put Student Nurse.  The hospitals needs all of this information in order to add you to their system and process your employee ID number.

New users will receive two emails:  one will contain your user name, employee ID number, and instructions on how to reset your password.  The second email contains a code that is one part of your temporary password (you will use it in conjunction with the last four digits of your SSN).

You will then go to password.comhs.org, choose “change password” and enter the username and temporary password to reset.  Your passwords are good for 90 days.
